Summary

We are seeking an Advanced Clinical Practitioner to join our team. In this role, you will play a vital part in enhancing patient care by utilizing your clinical expertise and skills to assess, diagnose, and manage patients effectively.

Responsibilities

Conduct comprehensive assessments of patients to determine their healthcare needs. Develop and implement individualized care plans based on patient assessments. prescribe medications and monitor patient responses to treatment. Provide education and support to patients regarding their health conditions. Collaborate with other healthcare professionals to ensure coordinated care. Maintain accurate and up-to-date patient records in compliance with regulations.

Requirements

Registered Nurse or equivalent qualification with advanced clinical training. Strong knowledge of anatomy and physiology relevant to patient care. Experience in medication administration and management of long term chronic conditions. Excellent communication skills for effective interaction with patients and colleagues. Ability to work independently as well as part of a multidisciplinary team.
