This is a fantastic opportunity for an experienced ACP to work within a supportive multidisciplinary team, delivering high-quality, patient-centred care in a busy primary care setting. You will work autonomously and collaboratively to assess, diagnose, and manage a wide range of clinical presentations, contributing to the smooth running of the practice and improved patient outcomes.

Responsibilities
Conduct thorough assessments of patients, including taking detailed medical histories and performing physical examinations.
Develop and implement individualised care plans based on patient needs and clinical guidelines.
Administer medications and monitor patient responses to treatment, ensuring adherence to safety protocols.
Provide education and support to patients and their families regarding health management and treatment options.
Collaborate with multidisciplinary teams to enhance patient outcomes and streamline care processes.
Maintain accurate and up-to-date patient records in accordance with legal and ethical standards.
Stay informed about advancements in clinical practice, participating in ongoing professional development activities.
Qualifications
A recognised qualification in advanced clinical practice or equivalent experience within a healthcare setting.
Strong knowledge of anatomy and physiology, with the ability to apply this knowledge in clinical decision-making.
Proven experience in patient care, demonstrating excellent assessment, diagnostic, and management skills.
Proficiency in medication administration, including understanding pharmacology and potential side effects.
Exceptional communication skills, both verbal and written, with the ability to engage effectively with patients and colleagues.
