As an ANP within Clapham PCN Ltd, you will work autonomously and as part of our multidisciplinary team to deliver advanced clinical assessment, diagnosis, treatment, and prescribing for a wide range of patient presentations. You will help enhance same-day access, manage chronic conditions, support proactive care initiatives, and contribute to service development across the network.
This role is ideal for a confident clinician who thrives in a collaborative environment and is passionate about delivering exceptional care in primary care settings.
Key Responsibilities
Provide advanced, autonomous clinical care including assessment, diagnosis, and prescribing.
Manage acute, urgent, and long-term conditions.
Support proactive care, long-term condition reviews, and personalised care planning.
Work closely with the wider MDT including GPs, pharmacists, social prescribers, nurses and HCAs/care co-ordinators.
Contribute to PCN DES delivery, quality improvement, and pathway development.
Maintain high standards of clinical governance and professional practice.
About You
Registered Nurse (NMC) with MSc in Advanced Clinical Practice or equivalent.
Independent Prescriber (V300).
Strong clinical decision-making skills and primary care experience.
Ability to work autonomously while contributing effectively to a wider team.
Committed to ongoing development and high-quality patient care.
