Job Description

Complete Duty, on the day, phone calls from 8am-12am (timings may vary dependant on the start time of shift), completing duty phone calls in their entirety, (including completing management plans, prescribing medication and making referrals) Routine and emergency home visits for patient reviews and queries for the remainder of the shift. Ensuring appropriate emergency GP visits to the homes. The enhanced service will utilise a multi-disciplinary team approach to proactive care, integrated case management and urgent care during core general practice hours of 8am to 6:30pm Monday till Friday. Improving patient care by maintaining dignity, respect, and confidentiality.



Patient surveys and ongoing feedback, with scope for improvements and changes to better the service. Trained to undertake examinations of the cardiovascular, respiratory, abdominal and neurological systems and undertake the following: observations, venepuncture, ECG, urine dipstick, male and female catheterisation, rectal examination, injections, insulin, dressings and wound care. Understand common conditions which would require medical input with a focus on UTIs, respiratory conditions, frailty, falls, pain, post-op care, diabetes, fractures, mental health, dementia, and delirium. Ability to interpret findings from investigations and uses this to make clinical decisions about care and treatment.



Promotes the use of advance care planning within the care homes as part of improving end of life care. The monitoring of stock levels of dressings, appliances and medications. Implement screening programmes and health promotion on vaccinations and immunisations. Ensure Infection Control and Prevention is adhered to by CQC standard.



Early recognition of disease progression to prevent an ambulance call-out or hospital admission. This would incorporate encouraging homes to undertake the National Gold Standards Framework. Co-ordinate between homes, GPs Community Health Teams (such as the local Discharge to Assess Team), ambulance trusts and secondary care to prevent hospital admissions. Identify and help support vulnerable adults within a safeguarding model.



To record clear and contemporaneous computer based consultation notes to agreed standards using EMIS. Making professional, autonomous decisions in relation to presenting problems, whether self-referred to referred from other health care workers within the service. Assessing the health care needs of patients with undifferentiated and undiagnosed problems. Screening patients for disease risk factors and early signs of illness.



In consultation with patients and in line with current disease management protocols, developing care plans. Providing counselling and health education. Recording clear and contemporaneous consultation notes to agreed standards. Identifying patients who are frail and high risk.



Routine visits to support, advise and facilitate training to all homes in the region. Training will be clinical skills and teaching. Teaching will be based on early recognition of sepsis, confusion, pressure ulcers, falls and the importance of rehydration. Ability to identify risk by determining any significant events and near misses.



Continuous personal development in line with current research and evidence-based practice. Ensuring their revalidation is in line with Bromley GP Alliance. As a non-medical prescriber (NMP), work within the scope of national and local protocols. This will include attending regular updates in line with revalidation requirements, monitoring the effects of medications, advising homes on the safe and storage and disposal of drugs.



In general, the post-holder will be expected to undertake all the normal duties and responsibilities associate with a ANP working within Primary Care.
