The Advanced Nurse Practitioner (ANP) will work autonomously within the primary care team to assess, diagnose, treat, and manage patients with a wide range of acute, urgent, and long-term conditions. The postholder will be an experienced and qualified advanced practitioner with the ability to independently prescribe and make complex clinical decisions. The ANP will support same-day access, chronic disease management, clinical leadership, and service development within the practice.
Key Responsibilities
Clinical Responsibilities
Provide same-day urgent care assessments (face-to-face, telephone, and video consultations).
Independently assess, diagnose, and manage patients presenting with acute illness, minor injury, long-term conditions, and undifferentiated symptoms.
Perform clinical examinations, order and interpret diagnostic tests, and make evidence-based decisions.
Prescribe medicines safely and appropriately, including deprescribing where required.
Manage long-term conditions such as diabetes, asthma/COPD, hypertension, heart failure, and other chronic diseases (depending on experience).
Support frailty, care home residents, and vulnerable patient groups.
Provide emergency and urgent care interventions, including escalation to GPs or secondary care as needed.
Undertake home visits, if required, to assess patients unable to attend the practice.
Offer health promotion, disease prevention advice, and lifestyle interventions.
Maintain high-quality, accurate clinical records in line with policy.
Operational & Leadership Responsibilities
Contribute to the efficient running of the urgent care/triage system.
Work collaboratively with GPs, pharmacists, paramedics, nurses, HCAs, and administrative teams.
Support QOF, IIF, practice audits, and clinical governance activities.
Participate in MDT meetings and complex case management reviews.
Contribute to safeguarding, infection control, and risk management processes.
Provide mentorship, clinical supervision, or training to colleagues and students where appropriate.
Support service development and quality improvement initiatives within the practice.
Qualifications & Requirements
Essential
Registered Nurse (NMC) with Master's-level Advanced Clinical Practice qualification or equivalent portfolio.
Independent Prescribing qualification (V300).
Significant post-registration clinical experience in primary, urgent, or emergency care.
Advanced skills in clinical assessment, diagnostics, clinical decision-making, and autonomous practice.
Excellent communication and interpersonal skills.
Ability to work independently and as part of an MDT.
Commitment to ongoing CPD and maintaining a professional portfolio.
Desirable
Experience within general practice or PCN roles.
Competence in chronic disease management (e.g., diabetes, COPD, CVD).
Experience using EMIS Web or similar clinical systems.
Experience supporting quality improvement or clinical audits.
Teaching, mentoring, or supervisory experience.
