Job Summary

We are seeking a skilled and compassionate Advanced Nurse Practitioner to join our healthcare team.

Responsibilities

Triage patient requests Handle on the day acute appointments Work alongside the Duty Doctor to handle on-the-day capacity Provide assessments, screening, treatment services, and health education advice Provide appropriate follow ups for patients Provide assistance to the Practice Nursing team Ensure accurate notes are recorded Attend and participate in practice meetings as required Assist in formulation of strategy and policy Undertake administrative procedures Participate in collection of stats for audits

Requirements

2+ years ANP qualification 2+ years primary care experience Ability to work to 10-minute appointments Experience & qualifications in chronic disease management Awareness of accountability of own role and other roles in a nurse-led service. Awareness of clinical governance issues in primary care. Knowledge of patient group directions and associated policy. Change-management skills and ability to support patients to change lifestyle. Good communication skills, both written and verbal. Ability to communicate difficult messages to patients and families. Negotiation and conflict management skills. EMIS Web clinical system experience. Management of long-term conditions. Involvement in implementing and using protocols and clinical guidelines. Experience of audit.
Job Type: Part-time

Pay: 27.00-30.00 per hour
