Job Description


The Advanced Nurse Practitioner (ANP) will be an experienced, autonomous clinician working within their professional boundaries to provide expert clinical care, including the authority to independently prescribe medications. The ANP will be an integral part of the total triage model, collaborating closely with triage GPs to manage patient flow, including telephone, online, and face-to-face consultations.
Main duties of the job
The role includes daily home visits to patients in care homes and private residences as part of the clinical rota. The ANP will manage acute presentations, long-term condition reviews, and coordinate care pathways across multidisciplinary teams.

Job responsibilities
Independently assess, diagnose, plan, implement and evaluate care for patients presenting with both undifferentiated and chronic conditions.
Work within the total triage model: reviewing online and telephone consultations, escalating appropriately and managing within scope.
Prescribe and review medications within scope of practice, aligned with local and national guidelines.
Conduct daily home visits to patients in care homes and in their own homes, providing acute and chronic condition management. Manage patients with complex needs using evidence-based practice. Interpret and act upon diagnostic results, correspondence, discharge summaries, and clinical alerts.
Initiate and monitor treatment plans, ensuring continuity of care. Support urgent/emergency presentations, initiating appropriate response pathways.
Maintain accurate, contemporaneous patient records and documentation in accordance with practice protocols. Communication & Coordination
Maintain effective communication with triage GPs, practice staff, community teams, and external stakeholders.
Use sensitive, culturally appropriate communication methods when consulting patients and families.
Act as a patient advocate, supporting informed choice and selfmanagement.
Participate in MDT meetings, safeguarding discussions, and palliative care coordination. Service Quality & Governance
Participate in clinical audits, significant event reviews, and quality improvement initiatives. Adhere to evidence-based guidelines (NICE, CQC, NHSE) and practice policies.
Recognise and act within own competencies and scope of practice as per NMC standards. #
