We are looking to recruit a

2x 0.5 FTE qualified

Advanced

Nurse Practitioner

(prescriber qualification needed ) to join our motivated, friendly and supportive team at the Fairlands Practice Guildford. The successful candidate will be confident in independently managing acute cases of minor illnesses arising from the clinical triage system. Seeing and reviewing patients with long term conditions. Willing to take part in leading, motivating, and training staff and students. Supporting the triage and care navigation of patients as well providing care to patients face to face, over the telephone and sometimes in their own homes.

We are looking for someone who:

Will act autonomously within their professional scope of practice providing care for patients face to face and via telephone consultations, ensuring clinical safety and excellence.

Is able to assess, diagnose, and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency including making use of skills in history taking, physical examination, problem solving and clinical decision making.

Will diagnose, manage, and review acute and chronic conditions, including drug and non-drug-based treatment methods into a management plan.

Will work as part of a multi-disciplinary team to deliver safe and high-quality care.

Will carry out clinical nursing practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.

Can deliver care in line with NSF, NICE guidelines and evidence-based care.

Can prescribe and review medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.

Qualifications and Experience:

Registered Nurse with Nursing and Midwifery Council.

Masters degree for qualification post December 2020 or Post graduate Diploma or Degree for Advanced Practice Qualification pre December 2020.

Qualified Independent Nurse Prescriber on the NMC register.

Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet.

Meets the standards for registered ANP working at advanced level.

Minor illness qualification.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

Applicants must have current UK professional registration.

Pay= 63,000 to 65,000 per annum pro rata
