We are seeking an Advanced Nurse Practitioner or Paramedic Practitioner, who will work as part of our same day team, which includes an allocated duty doctor each day, to provide telephone triage and clinical assessment throughout the day. You will therefore require experience and a suitable qualification in independently diagnosing and treating minor illnesses and injury with planning for onward referral where needed.
In order to practice independently a V300 or Independent/Supplementary prescribing qualification would be essential. This is an ever evolving position within general practice and we look forward to embracing your ideas and suggestions in developing our acute care services.

Our practice provides services for approximately 13,000 patients from a main surgery in Kidlington and a branch surgery in Yarnton.

Clinical Care
Act as an expert practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing or paramedic roles.
Make direct referrals to primary and secondary services within locally agreed pathways, guidance, and protocols.
Provide clinical advice to Reception staff where required.
Ensure that patients receive high quality clinical care, delivered in a timely manner.
As an Independent Prescriber, prescribe medicines within their competency level.
Ensure complete and accurate documentation of each patient contact.
Work as an independent practitioner and as part of the multidisciplinary and multi-agency teams
Manage documents and correspondence received via e-consult and action appropriately.
Management Responsibilities
Promote professionalism as per the Nursing/ HCPC Code of Conduct
Contribute to the development of an excellent learning environment.
Support revalidation - carry out reflective feedback on practice and professional development with colleagues and action any tasks that arise as a result of reflection.
Support the Nursing Team as needed, particularly if urgent care is required.
Attend regular meetings with team members and report back to clinical lead and practice manager.
Professional
Adhere to the NMC/ HCPC Code of Conduct and at all times work within the scope of professional practice.
Ensure that professional practice adheres to organisational policies, procedures, and guidelines.
Maintain a professional manner and act as a role model and mentor for junior staff including trainees and student nurses.
Maintain confidentiality regarding information pertaining to patients and staff.
Ensure that the service interfaces with all other departments in a professional and productive manner, providing an effective service to partner organisations and other service providers.
Keep up to date with mandatory training and revalidation requirements.
Communication
Communicate effectively with patients and families regarding complex information about their care.
Provide advice and information to patients, carers and their families where appropriate.
Communicate effectively and work collaboratively with medical, nursing and allied health professionals to ensure delivery of a coordinated service.
Ensure accurate documentation record keeping in line with local policies and NMC/ HCPC guidelines.
Promote and demonstrate effective communication networks within the organisation, with partner organisations and other service providers.
Clinical Governance
Work with the Clinical leads to investigate adverse clinical incidents and complaints.
Adhere to The Key Medical Practice procedures and guidelines.
Service Improvement
Contribute to quality improvement within the service through the identification and initiation of changes, which lead to better evidence-based practice and improved standards of care.
Participate in clinical audit, identifying areas of best practice and areas for improvement.
Contribute to the setting and monitoring of standards of care.
Help develop the services available to the urgent care population to reflect identified needs.
Act as a mentor /supervisor of staff as appropriate
