Job Purpose To provide advanced clinical care to patients presenting with a broad spectrum of health problems, within the primary care setting. The post holder will work autonomously within professional boundaries, utilising advanced clinical knowledge and skills to assess, diagnose, treat, and manage patients effectively. Key Responsibilities Undertake comprehensive clinical assessments of patients presenting with both minor and complex conditions Diagnose and manage acute and chronic illnesses independently and in collaboration with the wider team Prescribe and review medications safely and appropriately Request, interpret, and act on investigations such as blood tests Deliver same-day and pre-booked clinics for minor illness and long-term condition management Provide health promotion, lifestyle advice, and self-care education Support chronic disease management in collaboration with GPs and practice nurses Participate in clinical audits, quality improvement projects, and significant event reviews Contribute to the development of new services and pathways within the practice and Primary Care Network Maintain accurate and timely clinical documentation in line with practice standards Provide mentorship and clinical support to other nursing and healthcare staff Professional Standards Work in accordance with the NMC Code of Conduct and professional guidelines Maintain ongoing professional development and revalidation requirements Engage in clinical supervision and reflective practice Adhere to local and national governance, safeguarding, and infection control policies
