We are seeking a dedicated and compassionate Advanced Nurse Practitioner to join our healthcare team. The ideal candidate will be responsible for providing high-quality patient care, conducting assessments, diagnosing conditions, and developing treatment plans. This role requires a strong commitment to patient advocacy and the ability to work collaboratively within a multidisciplinary team to ensure optimal health outcomes.
1 Responsibilities for Patient Care
1.1 To uphold the values of The Mosslands Medical Practice (MMP)
1.2 In accordance with the MMP timetable, work plan and appointment templates, the post-holder will make him/her-self available to undertake a variety of duties including but not limited to surgery consultations, telephone consultations and queries, checking and signing repeat prescriptions and dealing with queries, results, paperwork and correspondence in a timely fashion
1.3 To triage and complete all home visits for patients, ensuring timely and effective housebound treatments.
1.4 Use critical thinking in the clinical decision-making process to make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
1.5 Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
1.6 Screening patients for disease risk factors and early signs of illness
1.7 In consultation with patients and in line with current practice disease management protocols, developing care plans for health
1.8 Provide counselling and health education
1.9 Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
1.10 Recording clear and contemporaneous consultation notes to agreed standards, collecting data for audit purposes
1.11 Ability to undertake remote and triage consultations using digital platforms in accordance with practice policy and patient needs.
1.12 Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
1.13 Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
1.14 In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with working alongside GPs within primary care
1.15 Completing reports as directed by the Practice Manager and/or Clinical Lead
1.16 Provide quality primary care in other settings.
1.17 Use population health data to identify and proactively manage patients at risk of deterioration or unmet need, in collaboration with the wider PCN team.
1.18 Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety.

Responsibilities for Human Resources
2.1 All staff will be familiar with and abide by HR policies and procedures to support the smooth running of the company and will contribute to adequate clinical cover across sites.

Responsibilities for physical and financial resources
3.1 Support MMP efforts to maximise practice income through schemes including QOF and Salford Standard
3.2 Support MMP efforts to appropriately control practice and NHS expenditure e.g. cost-effective prescribing, appropriate referrals, waste reduction.
4 Communication & Leadership

4.2 Undertake mentorship, teaching and supervision for staff of all levels including doctors, medical students, nurses, student nurses, AHPs and admin staff, assessing competencies against set standards.

12 Physical, Mental & Emotional Demands of the Post
12.1 This post can be mentally and emotionally demanding. The nature of this role may include dealing with vulnerable patients, patients with complex needs and/or difficult personal circumstances, exposure to extremes of emotion and psychological distress.
13 Personal & Professional Development
13.1 MMP encourages all staff to continually update and develop their skills and knowledge, as this ultimately leads to a more satisfied workforce and therefore better services for our population.
13.2 MMP participates in monthly practice half-day closures which staff are invited to attend.
13.3 MMP undertake annual appraisal in which each individual member of staff can articulate their objectives and aspirations, to develop themselves professionally and personally.
13.4 Each member of staff has a responsibility for their own development and that of others.
13.5 Engage with 1-2-1 meetings and NHS and MMP appraisal processes.
13.6 Contribute to colleague's appraisal and provide 360 feedback when requested and able.
14.
Supervision and Support
14.1 The post-holder will receive regular clinical supervision from the Clinical Lead or designated GP supervisor and will have access to protected CPD time as per PCN DES guidance

Qualifications
Advanced nursing degree (Master's or Doctorate)
Certification as a Nurse Practitioner (NP)
