About the Role



An exciting opportunity has arisen for an enthusiastic, committed, and caring Advanced Nurse Practitioner (ANP) to join our friendly, well-established two-site practice.

Working within the NMC Code of Conduct and using your advanced clinical skills and experience, you'll provide autonomous, high-quality patient care and play a key role in developing our nursing services.

You'll work closely with our experienced multidisciplinary team, seeing and managing a wide range of patients, including same-day triage cases, chronic disease management, and acute presentations.

The role will be based across both sites (Greens Norton and Weedon), so a full driving licence and access to transport are essential.

Key Responsibilities



Assess, diagnose, and treat patients autonomously, providing evidence-based, patient-centred care. Undertake same-day triage as part of the duty team, prescribing and referring as appropriate. Manage patients with undifferentiated and undiagnosed conditions. Support and promote long-term condition management and health promotion. Work collaboratively with GPs, nurses, and the wider primary care team. Contribute to service development, audits, and continuous improvement initiatives.

Requirements



Essential:



Registered General Nurse with current NMC registration Independent prescriber Master's degree in a relevant area Previous experience in general practice Full driving licence and access to transport Strong clinical decision-making and risk management skills

Desirable:



Experience with SystmOne Chronic disease management training Involvement in audit or research

Pay: 43,680.00-49,920.00 per year
