Job Description

Main duties of the job To support and work alongside the GPs and clinical team to deal with a variety of long standing conditions, as wells as acute/on-the-day patients including triage. This role includes assessment, diagnosis and treatment of these patients who present with undiagnosed conditions. Patient contacts may be face to face, over the telephone or electronic. The post holder will provide and maintain a high standard of care for patients as well as providing support to the GPs and other members of the primary health care team.

This position may very occasionally also include visiting housebound patients to assess, diagnose and treat when they are unable to access their practice.
