We are seeking a dedicated and compassionate Advanced Nurse Practitioner to join our healthcare team as part of the PCN, working full time at Willow Green Surgery on a fixed term contract of 6 months. The ANP will play a vital role in providing high-quality patient care, supporting the clinical team, and ensuring that patients receive the best possible treatment. This position requires a strong understanding of anatomy and a commitment to patient care, making it ideal for individuals who are passionate about improving health outcomes in the community.
Duties
Conduct patient assessments and take medical histories to ensure comprehensive care.
Administer vaccinations and injections as per clinical guidelines.
Assist in the management of chronic diseases by monitoring patients' conditions and providing education on self-management.
Experience in long term condition reviews
Perform routine clinical tasks such as wound care, dressings, and suture removal.
Collaborate with doctors and other healthcare professionals to develop and implement patient care plans.
Maintain accurate patient records and documentation in accordance with legal and ethical standards.
Educate patients on health promotion, disease prevention, and healthy lifestyle choices.
Participate in quality improvement initiatives to enhance service delivery within the practice.
Requirements
Registered Nurse qualification with valid nursing registration.
Strong knowledge of anatomy and physiology.
Proven experience in a clinical setting, preferably within primary care or community health.
Excellent communication skills with the ability to build rapport with patients.
Strong organisational skills and attention to detail.
Ability to work effectively both independently and as part of a multidisciplinary team.
