The Role
We're excited to welcome an Advanced Clinical Practitioner to join our close-knit team on a part-time basis. This is a fantastic opportunity for someone who enjoys building relationships with patients, working collaboratively, and making a real difference within the local community.
You'll play a key role in delivering advanced clinical care while being fully supported by experienced GPs, nurses, and the wider practice team.
What You'll Be Doing
Carrying out advanced clinical assessments and managing patients with a wide range of health needs
Delivering high quality, patient-centred care in line with practice guidelines
Working closely with GPs, nurses, and other healthcare professionals as part of a supportive multidisciplinary team
Maintaining accurate and timely patient records
Supporting patients with health education, self-management, and preventative care
Taking part in ongoing professional development to keep your skills and knowledge up to date
What We're Looking For
Experience working as an Advanced Clinical Practitioner (or similar) within primary care
Strong clinical knowledge and sound decision-making skills
Excellent communication skills and a collaborative, team-focused approach
Confidence working independently while knowing when to seek support
Registration with the relevant professional regulatory body
Experience with medication management (desirable, but not essential)
A genuine passion for delivering kind, compassionate care

Friday:
8:30am - 5:30pm
