About the Role





Tendring PCN is looking to recruit an advanced clinical practitioner/specialist nurse to join our multi-disciplinary team, to help us to transform and modernise pathways of care, enabling the safe and effective sharing of skills across traditional professional boundaries. This position is offered on an 18 month fixed term contract with working patterns and locations to be discussed at interview. We have opportunities available to work within and/or across our 3 practices: Great Bentley Surgery, St James Surgery and Mayflower Medical Centre.



All areas of special interest are welcomed, with a keen interest inchronic disease management.



The post-holder will:



Demonstrate a high level of expertise within the specialist service providing advice, education and support to staff, patients, their families and carers. Be practising autonomously as an advanced clinical practitioner within the designated speciality area to provide patient-centred clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the directorate. Support new ways of working that emphasises a more efficient and patient-focused service, and will ensure the safe treatment, referral and discharge of patients with undifferentiated and undiagnosed presentations in their area. Have responsibility for leading the on-going development of clinical practice and standards of care within the service, including the development of policies, procedures, protocols and guidelines in collaboration with multidisciplinary colleagues.

About the Candidate




To be considered for this position you will require the following qualifications, skills and experience:


Professional registration with relevant professional body: NMC Post-graduate qualification in specific speciality, or equivalent experience Post-registration experience at a senior level (Band 7 or above) in specialty area Evidence of on-going CPD Advanced clinical assessment skills Excellent communication skills Ability to perform in stressful and complex situations Sound clinical reasoning & decision making Flexibility Ability to inspire and lead a team

Great Bentley Surgery, Great Bentley (GBS) St James Surgery, Clacton (SJS) Mayflower Medical Centre, Harwich (MMC)

Our aim is to deliver better health outcomes for our local patient populations, and reduce health inequalities, via provision of additional roles and services to extend the General Practice Teams.



Tendring PCN are one of the 22 PCN Test Sites as part of the national PCN pilot programme, a partnership with SNEE ICB and NHS England to narrow demand and capacity gaps, improve workforce and patient experiences, as well as sustain general practice for the future.
