. The ACP will play a vital role in delivering high-quality patient care, providing advanced assessment, diagnosis, treatment, and management of patients in collaboration with the wider primary care team.
Key Responsibilities
Conduct comprehensive clinical assessments, including taking patient histories and performing examinations.
Diagnose and manage acute and chronic conditions, prescribing medications where appropriate.
Develop and implement individual care plans in collaboration with patients and other healthcare professionals.
Provide autonomous decision-making within the scope of professional practice and regulatory requirements.
Offer expert clinical advice and support to patients, carers, and colleagues.
Contribute to the delivery of health promotion and disease prevention initiatives.
Liaise with multidisciplinary teams to ensure holistic patient care.
Maintain accurate and timely patient records in line with regulatory and practice standards.
Stay updated with current clinical guidelines and best practices.
Participate in audits, training, and professional development activities to support continuous improvement.
