We are seeking a dedicated and skilled Clinical Pharmacist to join our Primary Care Network APOLLO. In this role, you will work closely with healthcare professionals to deliver high-quality, effective, and efficient medicines management services to our patients. We are a supportive and collaborative PCN, committed to developing our staff to their full potential.Main duties of the jobAs a Clinical Pharmacist, your key responsibilities will align with the Primary Care Network Directed Enhanced Service (PCN DES), specifically focusing on the following:

  • Medication Reviews
  • Support for Long-Term Conditions
  • Prescribing and Safety:
  • Provide patient Education and Support:
  • Contributing to PCN Initiatives and Targets:
  • Support Audit and Quality Improvement:
  • Medicine Information and Staff Support:
  • Provide medicine-related information and training to practice staff to improve prescribing practices and ensure accurate medication administration.
  • Act as a key resource for the practice team, providing guidance on medication supplies, doses, side effects, and any other medicine-related queries.

  • Conduct structured medication reviews for patients with complex polypharmacy, implementing prescribing decisions based on review outcomes, and requesting appropriate tests.
  • Perform medication reviews (face-to-face, remote, and e-consultations, home visits, care homes), proactively managing patients to improve medication adherence and outcomes, particularly for patients with long-term conditions or multi-morbidity.
  • Support for Long-Term Conditions:
  • Work with the team to support long-term condition reviews, offering medicines management advice for optimal patient care.
  • Conduct care home ward rounds, providing medication reviews and supporting safe de-prescribing where appropriate.
  • Prescribing and Safety:
  • Ensure that all medicines prescribed are aligned with the most up-to-date clinical guidelines and are safe, effective, and cost-effective.
  • Monitor and audit prescribing practices, promoting cost-effective prescribing across the PCN.
  • Ensure that drug safety and safety monitoring protocols are adhered to, conducting regular safety audits and promoting drug safety across the network.
  • Patient Education and Support:
  • Provide education to patients regarding their medications, supporting patients to get the best outcomes from their medicines.
  • Proactively address medication-related issues and offer advice to both patients and healthcare professionals, acting as the interface with community and hospital colleagues.
  • Contributing to PCN Initiatives and Targets:
  • Contribute to the achievement of high IIF (Improvement and Impact Fund) and QOF (Quality and Outcomes Framework) targets, particularly focusing on medication-related indicators.
  • Participate in multi-morbidity reviews and contribute to multidisciplinary team reviews to optimize patient care.
  • Collaborate in PCN initiatives to improve service provision, contributing to service improvements within enhanced services such as QOF and IIF.

  • Independent Prescriber or working towards it
