Job Introduction

Are you passionate about improving the quality of life for children and young people? Do you have a keen interest in supporting education, health, and care services? If so, we have an exciting opportunity for you to join our SEND Assessment and Review Team as an Assistant Education Health and Care Co-Ordinator.


In this key role, you will support a caseload of children and young people, working collaboratively with various partner agencies and organisations both internally within BCP and externally. You will support case-holders with the monitoring of timescales and processes associated with Education Health and Care assessments and the Annual Review cycle for existing Education, Health and Care Plans (EHCPs) in line with statutory guidance. You will also oversee phase transfer cohorts, with the support of the Education, Health and Care Co-Ordinator, ensuring the best outcomes for children and young people in your area.


Key Responsibilities:

Support processes that deliver against statutory timeframes as governed by the SEND Code of Practice and other legislative frameworks. Assist families in understanding processes regarding Annual Reviews and other processes affecting children and young people with an EHCP. Gather information to support decisions about maintaining EHCPs. Manage and update files on the Caseload Management system, ensuring accuracy and compliance with standard procedures. Promote and enable the inclusion of children and young people with SEND in collaboration with colleagues, partners, and parent/carers. Prepare briefs for advisory panels, ensuring information is accurate and complete. Build knowledge of local providers to meet the educational, social, and health needs of children and young people. Engage with a range of professionals, including educational psychologists, social workers, health service staff, and training providers. Provide efficient administrative support, including processing correspondence, emails, and phone calls. Liaise with SEND teams of other Local Authorities and the Admissions team to manage EHCPs for children moving in or out of the local authority. Ensure services are planned and delivered in line with local and national priorities. Deliver continuous service improvement with a focus on improved outcomes for children, young people, and their families.

As a result of the creation of BCP Council in 2019, we are working on simplifying our terms and conditions of service and will be creating a single pay structure which is to be applicable for all employees.
