How you'll make a difference:
As an Assistant Education Health Care and Plan Coordinator, you will work within a small cluster team and be the main point of contact for parents and carers of children and young adults in relation to their Education Health Care Plan (EHCPs) or assessment. Your role is crucial in supporting the parents and carers in making sure they're aware of any progress and developments.
The Education, Learning and Skills department provide high quality, coordinated child, young person and family-centred services which respond to needs and adopt a preventative and early help approach. Whenever and wherever possible, services will be provided at a mainstream universal level and barriers to this will be removed.
What you will be doing:
On a rota basis you will offer support, advice and guidance to the SEND community.
Working alongside schools and relevant professionals, you will develop strong relationships with those involved in annual reviews and new assessments, ensuring reports and information are received in a timely manner.
You are responsible for ensuring the work of the cluster team is on track by monitoring timescales, updating progress spreadsheets and keeping everyone informed about possible delays or missed deadlines.
It will be key for you to attend annual review meetings, amending and finalising EHCPs as a result, while liaising with parent/carers and professionals in order to produce an improved and updated plan that meets our quality assurance standards.
It will be your responsibility to follow up with students who are no longer in education, employment or training and ensure they have the correct advice and support, ceasing EHCPs where appropriate.
What we need from you:
You will have a relevant qualification e.g. NVQ Level 3 or relevant experience in health or social care or equivalent.
Given the nature of this role, you will have previous experience of working with a broad range of people including families, education, social care and health care professionals.
It is essential you possess excellent IT skills both in Office 365 and to use the client database.
Ideally, you will have experience and understanding of or be prepared to work in accordance with the local government authorities safeguarding policies and procedures.
What you need to know:
This role is being offered as either:
	+ 25hrs per week, working all year round
	+ 30hrs per week, term time only, working 39 weeks per year.
This role is hybrid, with a mixture of working at our office in Badminton Road, Yate and from home.
Please note that due to the nature of this role, it is a requirement of employment that an enhanced Disclosure and Barring Service (DBS) check is obtained.
You will be expected to travel throughout the authority and surrounding area so you must have a full, valid driving licence with regular access to a vehicle or have an alternative means of travel.
