Job Description

Job Purpose To work as an integral member of the Adult Speech and Language therapy team, providing face-to-face rehabilitation to clients with swallowing and communication difficulties in community settings. To deliver communication group therapy and training to others, including care home staff, with support and supervision from qualified clinicians. To support to the speech and language therapists by preparing therapy resources, such as communication picture aids, and collating medical information from medical notes, patients and carers to support with the triage process. Dimensions To work in a variety of community settings which may include a patients own home/residential home/nursing home and out-patient clinics setting up assessment, treatment including rehabilitation programmes for patients, carers, and families.



To participate actively in the services training strategy, including in-service training. Trust and multi-agency training. Key Responsibilities Clinical 1. To undertake individual treatment and interventions with patients under the supervision of qualified staff.



To review clients and adapt individual and group exercise programmes as appropriate 1. To refer service users to external agencies and specialists when their needs are beyond the assistant practitioner role 2. To accept responsibility for a designated caseload, working independently and taking an active role in promoting independence and meeting the needs of the service user. 3.



To promote well-being, safe mobility, transfers and effective communication. To provide instruction, advice and intervention to service users and carers in line with the team guidelines. 4. To assess, provide, fit and demonstrate use of a wide range of equipment to promote independence.



5. To attend and contribute to team meetings and case discussions using effective communication. 6. To lead groups and carry out training packages under the supervision of qualified clinicians 7.



To maintain comprehensive and accurate records in line with employers polices Managerial and Leadership 8. To provide support to other Assistant Practitioners as appropriate. 9. To feed into and support team service improvement initiatives Education and Development 10.



To ensure knowledge, skills and competencies are kept up to date and complete all relevant competencies within recognised time frames (as detailed in the competency guidelines). 11. To be involved in formal appraisal systems, supervision, and be active in identifying aims and objectives for personal development in line with departmental policy. Clinical Governance 12.



To maintain a safe working environment and be aware of the necessity to comply with health and safety regulations. 13. To have regard at all times for the confidential nature of the work by adhering to confidentiality legislation and not to disclose information to unauthorised parties. 14.



To actively promote equality and diversity within service delivery and peoples individual rights. 15. The post holder will comply with current legislation, the organisations policies, procedures and core values. This is not an exhaustive list of duties.
