37 hours per week
We are seeking to recruit a highly motivated and experienced individual who will be responsible for a core group of individuals within a range of support needs; Learning disability, physical disability, complex behaviours, neuro diverse and/or individuals with a diagnosis of Dementia.
The post holder will support the Team Leader in the delivery and monitoring of individuals service to promote a personalised, and person-centred service that responds to individual's needs and preferences.
A focus will be on person centred and person led services; you will be expected to develop opportunities for people and their families in relation to outcomes identified in the individuals Care and Support Plan.
The role will involve working with other professionals to ensure statutory requirements and strategic direction are met and deliver outcomes that are driven by the Social Services & Wellbeing Act (Wales) 2014
You will be responsible for the direct leadership of a small core group of staff and support other core groups in the absence of their ATL.
