to support adults across the borough under a strengths-based model of practice. You will hold a caseload of non-complex cases, carrying out assessments, reviews and support planning to help residents maximise their independence, wellbeing, resilience, and engagement with community resources.
You will work closely with qualified Social Workers and multi-disciplinary teams, contributing to improved outcomes and ensuring residents receive the right support at the right time. This role is vital in delivering high-quality adult social care and supporting safe, effective and person-centred decision making.
Key Responsibilities

Casework & Support
Hold a caseload of non-complex cases, supporting adults to improve independence, resilience and wellbeing.
Undertake strengths-based

Provide practical support, including helping service users access: Living aids and equipment, Welfare benefits, Community resources.
Assist in arranging short-term care and respite to help prevent carer breakdown.
Direct Work with Adults & Carers
Work with adults, families, carers and communities to help them make informed decisions and engage in their support plans.
Develop effective working relationships to motivate and support individuals.
Multi-Agency Working
Liaise with colleagues across the health and social care partnership.
Attend MDT meetings and contribute towards achieving positive outcomes.
Attend safeguarding strategy meetings as required.
Record Keeping & Compliance
Maintain accurate, timely and complete case notes, documentation and reports.
Update systems and contribute to information gathering for legal purposes where required.
Undertake regular reviews of care packages.
Complete
Continuing Health Care (CHC)
checklists and identify cases requiring full assessment.

Essential Skills & Experience
Genuine motivation to develop a career in Adult Social Care.
Understanding of how Adult Social Care supports vulnerable adults to maximise independence.
Ability to explain and apply social care values, ethics and strengths-based practice.
Demonstrated achievement focus and reliability in previous work or voluntary experience.
Strong understanding of diversity, equality and cultural awareness.
Competent in using IT systems for case recording and communication.
Enhanced DBS required
Desirable
Experience supporting adults in a social care environment (voluntary or paid).
Knowledge of local community resources and support networks.
