22.5 Hours per week

We provide complex care to vulnerable individuals, often in challenging environments, ensuring they receive high-quality support closer to home. As part of our commitment to meeting local needs with available resources, we are seeking an enthusiastic professional ready to make a real difference.

Main duties of the job
As a Community Nurse you will care for patients and their families in the way that you believe is best, delivering our high service standards, sharing best practice, and actively improving the way we work to help us continue to change things for the better. As a Band 5 Community Nurse, you'll be part of our valued team at Five Rivers Health Centre, working for the City Community Team.

You will be a NMC/1stlevel registered Nurse, you will have the ability to work both independently and also as part of a wider multidisciplinary team. You will have a full UK driving license with access to a car for work purposes.
