What you will do:
- Coordinate and deliver community nursing care as part of the Continuing Healthcare Team
- Assess, plan, and manage patient care autonomously while maintaining high standards
- Collaborate with multi-disciplinary teams to ensure holistic patient care
- Provide support and guidance to junior staff and trainees, fostering a learning environment
- Engage in continuous professional development to enhance practice
Essential Criteria:
- Experience as a Senior Nurse
- Meeting associated essential criteria of a standard Band 6 nurse
- Must possess a valid UK driving licence
Desirable Criteria:
- Experience in Continuing Healthcare
