Job Description

We are looking to recruit a motivated Band 6 Therapist who relishes a fast paced and ever changing environment, to join the Home First Team, based at St Peter's Hospital, Chertsey.



The Home First Team covers the "front door" of the hospital:

Accident and Emergency, Clinical Assessment Unit and Older Persons Assessment Unit (OPAU) and brings together acute hospital services with Adult Social Care and District and Borough Services whilst maintaining strong links with the Urgent Community Response Team.



The role is supported by senior colleagues and there is a requirement to work closely with other members of the multidisciplinary team to provide timely, holistic and comprehensive therapy assessments, facilitate rapid discharge of medically optimised patients and avoid unnecessary hospital admissions.



There will also be the expectation supervision and development of junior staff will be embraced, contribution to the Therapy wide training programme offered, along with contributing to service improvement initiatives and clinical governance activities.



ASPH offers a seven-day service across some teams and you will be expected to participate in seven day working as per service requirements. Flexible working requests will be considered.



To work as an autonomous practitioner and manage a specialist caseload including using respiratory, medical, neurological, and orthopaedic skills

To complete comprehensive therapy assessments and formulate suitable interventions and discharge plans to prevent hospital admissions

To provide education and teaching to junior therapists and the wider MDT acting as a specialist resource

To supervise junior staff, University and Return to Practice students and Apprenticeships from both Physiotherapy and Occupational Therapy disciplines

To actively participate in service development and contribute to monitoring of standards of practice
