Job Description

The Specialist Sister contributes to the provision of specialist palliative care in the North Camden community for patients facing life-limiting illnesses (within the remit of the multi-disciplinary team) and supporting the Clinical Nurse Specialists to manage their caseload. They will, with supervision, provide advice, education and support to patients, their carers and other health and social care professionals. They will contribute to the development and smooth running of the service. The post holder will work to agreed professional, departmental and Trust boundaries as required to provide patient centered seamless care and take part in the 7 day / week service to both community and

hospital patients.

The post holder will be expected:

To contribute, with clinical supervision, to the management of a caseload and facilitate the provision of palliative care and end of life care to patients with life limiting issues, and potential facing their end of life, and support them and their families. To contribute to leadership and innovation of nursing practice in palliative care both in the community of North Camden, The Royal Free London Hospital, and through the local end of life locality group To contribute to the weekly specialist palliative care MDT To contribute to education particularly with respect to end of life care.
