Band 7 Community Mental Health Nurse - Job Opportunity  
We are seeking an experienced Band 7 Community Mental Health Nurse to join our dynamic team, delivering advanced clinical care and leadership within the community mental health setting. This role is pivotal in supporting complex case management and promoting recovery-focused care.
Key Responsibilities:
Undertake specialist mental health assessments and lead the formulation of person-centred care plans for individuals with complex needs.
Deliver high-level clinical interventions, including crisis resolution, risk assessment, and risk management planning.
Provide clinical leadership and supervision to junior staff, students, and support workers, ensuring high standards of care delivery.
Prescribe and monitor treatment regimens in line with NMP (if qualified), and oversee the safe administration and evaluation of medication.
Act as care coordinator under the CPA framework, ensuring timely reviews and multidisciplinary collaboration.
Liaise effectively with GPs, psychiatrists, social care teams, and voluntary agencies to coordinate integrated care pathways.
Contribute to service development, quality improvement initiatives, and audits in line with Trust objectives.
Ensure clinical documentation meets CQC and NHS governance standards, and participate in safeguarding procedures and serious incident reviews.
