To provide expert leadership and management within the All Age Continuing Care (AACC) service. The post-holder will be responsible for the comprehensive assessment, coordination, and case management of individuals with complex health needs, leading on the determination of eligibility for NHS Continuing Healthcare (CHC) and the commissioning of bespoke care packages. The role requires advanced clinical knowledge, leadership skills, and a robust understanding of the legal frameworks surrounding complex care. 2.
Main Duties and Responsibilities Clinical Leadership and Complex Case Management: Act as the lead professional for a defined caseload of individuals with highly complex, intense, and unpredictable healthcare needs. Autonomously manage the end-to-end CHC process, from referral to case resolution, ensuring all decisions are robust, defensible, and person-centred. Chair and facilitate Multi-Disciplinary Team (MDT) meetings, ensuring the effective use of the Decision Support Tool (DST) to reach legally sound eligibility recommendations. Professional and Team Supervision: Provide professional leadership, clinical supervision, and mentorship to Band 6 Assessors and support staff within the team.
Act as a source of expert advice on the National Framework for CHC and NHS-Funded Nursing Care for colleagues, health partners, and local authorities. Care Planning and Commissioning: Develop, implement, and regularly review highly specialist, personalised care and support plans for individuals eligible for CHC. Lead on commissioning tailored packages of care, ensuring they are clinically effective, cost-efficient, and meet the individuals assessed needs. Manage the financial aspects of each case within a delegated budget.
