Band 7 Nurse – Proactive Primary Care Project

Job Description

Help Lead the Future of Proactive General Practice

Are you a proactive problem solving nurse who believes primary care should anticipate problems rather than react to them?

Spirit Primary Care Ltd has launched a pioneering Proactive Primary Care (ProPC) programme designed to transform how people with long-term conditions are supported in general practice. This is a rare opportunity for an experienced nurse to lead a new model of care, combining clinical leadership, digital innovation, and population health thinking.
