Spirit Primary Care Ltd has launched a pioneering Proactive Primary Care (ProPC) programme designed to transform how people with long-term conditions are supported in general practice. This is a rare opportunity for an experienced nurse to lead a new model of care, combining clinical leadership, digital innovation, and population health thinking.
You will work across seven GP practices as part of a digitally enabled pilot focused on improving outcomes for patients living with conditions such as diabetes, anxiety and depression, hypertension and COPD reducing unplanned demand on primary care and improving quality of life through early, personalised intervention.
About the Role
As a Band 7 Proactive Primary Care Nurse, you will act as the clinical lead for proactive long-term condition management across multiple practices.
This is not a traditional practice nurse role. You will work autonomously and collaboratively, using data, digital tools, and advanced nursing skills to identify risk early, intervene sooner, and support patients to self-manage more effectively.
The role is mobile and partly remote, working closely with GPs, practice nurses, care coordinators, and wider healthcare teams.
What You'll Be Responsible For
Leading structured, proactive reviews for patients with complex or multiple long-term conditions
Undertaking advanced clinical assessments (e.g. blood pressure, spirometry, ECGs, phlebotomy, BMI)
Interpreting findings, making clinical decisions, and escalating or referring appropriately
Designing and overseeing personalised care and prevention plans
Supporting patient education, motivation, and self-management
Using remote monitoring and digital platforms to track progress and intervene early
Ensuring high standards of clinical documentation, safety, and governance
Working across multidisciplinary teams to deliver coordinated, joined-up care
Travelling between seven GP practices as required (mileage reimbursed)
Who We're Looking For
You will be:
Firstly a proactive problem solver who cares passionately
A registered nurse (NMC) with senior / Band 7-level experience
Experienced in general practice or community long-term condition management
Clinically confident managing patients with complex, multi-morbid needs
Comfortable working autonomously and making clinical decisions
A natural leader who enjoys shaping new ways of working
Passionate about prevention, early intervention, and population health
Confident using digital systems such as EMIS Web or SystmOne
An excellent communicator who works well across clinical and non-clinical team
V300 Prescriber status is desirable but not essential.
