Our team includes a Clinical Director, PCN Manager, Operational Manager, Mental Health Team, Advanced Clinical Practitioners, First Contact Physiotherapists, General Practice Assistant, Clinical Pharmacists, Care Coordinators and Social Prescribers, working collaboratively to deliver person-centred care and support population health improvement.
Job Summary
We are seeking a dedicated, experienced and compassionate Primary Care Practice Nurse to join our healthcare team. The successful candidate will be responsible for providing comprehensive medical care to patients across a wide range of health issues, promoting wellness, and managing chronic conditions. This role offers an opportunity to make a meaningful difference in patients' lives through expert clinical practice and personalised patient care. The position is suitable for qualified medical practitioners committed to delivering high-quality healthcare services.
This role will work closely with our newly appointed Enhanced Nurse focus on delivering high-quality, proactive care to patients with long-term conditions and supporting the PCN's wider health inequality and population health initiatives.
You'll work across PCN practices and within local communities, using data-driven approaches to identify need and target resources effectively.
Responsibilities
Conduct thorough patient assessments, including medical histories and physical examinations.
Diagnose and treat a variety of acute and chronic illnesses, ensuring appropriate management plans are implemented.
Managing and supporting patients with long term conditions (CVD/Diabetes/Respiratory conditions)
Provide preventative care advice, health education, and lifestyle counselling to optimise patient wellbeing.
Assess medications and therapies in accordance with current clinical guidelines.
Collaborate with other healthcare professionals to coordinate comprehensive patient care.
Maintain accurate and detailed medical records in compliance with legal and organisational standards.
Stay updated with the latest medical research, anatomy knowledge, and clinical practices to ensure evidence-based treatment.
Participate in continuous professional development activities to enhance clinical skills and knowledge.
Contributing to quality improvement and service development across the PCN
Key note: This is an exciting new role within our network, offering a unique opportunity for the right candidate to shape and evolve the position from the ground up working alongside our newly appointed PCN Enhanced Nurse. Robust clinical and operational support will be provided, the successful applicant will be empowered to lead the development of the role and drive its strategic direction.
It is expected that all primary responsibilities will be met by the six-month mark and sustained thereafter with clear benchmarks for success. To ensure a strong foundation, the appointed candidate will benefit from a well-supported induction phase, including dedicated time for administration, planning, and developmental activities.
Person Specification
Essential:
Registered Nurse (NMC)
Significant experience in primary care nursing
Strong understanding of long-term condition management
Excellent communication and teamworking skills
Confident in using EMIS/SystmOne
Desirable:
Experience in community outreach or health inequalities work
Experience supporting junior staff
Familiarity with Core20PLUS5 priorities
