Job Description

The Mount View Practice in Fleetwood are looking for an energetic, dedicated, caring and patient focused individual with a can-do attitude, who has the desire to make a difference to the lives of the patients in our community.
The HCA will be working under the direct supervision of our Practice Nurses and strictly in accordance with specific Practice guidelines and protocols.
The HCA will assist the Practice Clinical Team in the provision and delivery of prescribed programmes of patient care.
Ideally you will have the skills for a Primary Care environment including competency in phlebotomy and working with patients with long term condition such as diabetes.
This is a Bank position to cover Maternity.
Must be able to work Mon to Fri between the hours of 0800 and 1830.
Main duties of the job
New Patient Medicals
NHS Health Checks
Phlebotomy
Weight Management
Pre diabetic checks
Electrocardiogram
Annual reviews of long term conditions
Carry out urinalysis and preparation of specimens for investigation by the pathology laboratory
Clear and restock clinical consulting rooms
Ensure vaccines and cold chain storage monitoring and recording is carried out as per policies and protocols
Prepare and maintain environments and equipment before, during and after patient care interventions including assisting GPs during the performance of minor operations
Help to raise awareness of health and wellbeing and how it can be promoted
Act as chaperone when required
Home Visits
Where appropriate, provide support and encouragement to Patients to self-manage their conditions

The Mount View Practice is a patient focused, friendly team looking after approximately 12,000 patients in Fleetwood.
We consist of 5 GP Partners and a Salaried GP, a Practice Paramedic, Advanced Nurse Practitioners, Practice Nurses and Healthcare Assistants and a dedicated team of Administrative professionals.
We are part of Fleetwood Primary Care Network and supported by a fantastic team of Clinical Pharmacists, Pharmacy Technicians, Care Coordinators and Social Prescribers.
