Farleigh Hospice is a high performing organisation providing specialist palliative and end of life care for people with life limiting illness in the Mid Essex region.Our team at Farleigh has grown and evolved to meet the changings needs of the community we serve.
We are now looking for highly motivated Health Care Assistants to join our Bank on our Community Team & Inpatient Unit.
Main duties of the job
You would have experience and qualifications in the health care field. We welcome applications from people who have a passion for providing excellent care and support to our patient group. You will need a driving licence and access to a car for the community role.
About us
The 10-bed unit will provide holistic care for patients with both malignant and non-malignant life limiting illnesses and their families and carers. This will include excellent symptom control, psycho-social and spiritual support for patients who are admitted for symptom control, specialist palliative rehabilitation and end of life care; we are also aspiring to provide care for patients with complex nursing needs such as non-invasive ventilation and tracheostomy care.

As a Health Care Assistant you will work within a multi-disciplinary team providing clinical care and support to hospice patients and their families
To maintain effective communication skills to deliveremotive information clearly, sensitively and unambiguously with staff, patients, families and professionals by face-to-face, telephone and/or electronic means.
To work collaboratively as part of the multi-disciplinary team (MDT) team sharing the clinicalcaseload of patients/carers within the locality/in-patient unit for patients who have palliative care needs to ensure efficient and timely care is delivered.
To work as part of the MDT,seeking advice from colleagues and/orclinicalleadershipteam when required, in ordertodeliver excellentpalliative and end of life care.
To support and participate in any streamlined locality/IPU processes and services by working to agreed quality improvement standardsincluding clear outcome measures to produce efficient and highly performing clinical services.
To support the Hospice in building strong relationshipswith external partners and stakeholders and promote the good reputation of the Hospice e.g. local trusts, CCG, health and social care partners, businesses and voluntary organisations.
