Job Description

Our client is a gentleman in his mid 20's residing at home in Aylesbury. He has experienced a brain injury as a result of a road traffic accident, which has resulted in some cognitive difficulties and fatigue. He requires compassionate and reliable support to help him maintain his independence and well-being within his home and the local community.

Purpose of the Role:



To provide person-centred support and prompting to our client with his daily living tasks, and community engagement, enabling him to live as independently and fulfilling a life as possible. This role requires patience, understanding, and a respectful approach to supporting someone with cognitive challenges. There will also be requirements to assist him with safely navigating his way through the day and managing risks associated with his conditions. Personal care assistance may be required in certain circumstances, although this would not be the main focus.

Key Responsibilities:



Personal Care Support:



Providing gentle prompting and support with personal hygiene tasks such as showering/bathing, preparing meals, dressing, and grooming. Assisting with medication reminders as directed by healthcare professionals. Supporting with toileting needs with dignity and respect.

Home Support:



Providing prompting and assistance with light household tasks, such as tidying, meal preparation (following instructions), and laundry. Ensuring a safe and comfortable living environment.

Community Support:



Accompanying the client on outings in the local community, such as shopping, appointments, or social activities. Providing encouragement and support to engage in community-based activities. Assisting with navigating public transport or accompanying in their own vehicle (if applicable and insured).

Cognitive Support:



Providing consistent prompting and reminders for daily routines and tasks. Using clear and simple communication, adapting to the client's pace and understanding. Supporting the client with memory aids and strategies as recommended by healthcare professionals. Maintaining a calm and supportive environment to minimize confusion and anxiety.

Communication and Record Keeping:



Maintaining clear and concise records of support provided. Communicating effectively with family members and relevant healthcare professionals regarding the client's well-being and any concerns. Reporting any changes in the client's condition or needs to the appropriate individuals.

Person-Centred Approach:



Treating the client with dignity, respect, and empathy at all times. Respecting the client's choices and preferences. Encouraging the client's independence and promoting their well-being.

Person Specification:



Essential:



Previous experience in a care or support role, preferably supporting individuals with cognitive difficulties or brain injuries. A genuine passion for supporting others and a commitment to person-centred care. Excellent communication and interpersonal skills, with the ability to communicate clearly and patiently. Ability to follow instructions and work independently. Reliable, punctual, and trustworthy. Respectful of confidentiality and professional boundaries. Understanding of the challenges faced by individuals with cognitive impairments. Willingness to undertake relevant training. Full driving license and access to a vehicle (may be required for community support). Enhanced DBS check (or willingness to obtain one).

Desirable:



Formal care qualifications (e.g., NVQ/QCF in Health and Social Care). First Aid certification. Experience in supporting individuals within their own homes and in the community. Knowledge of local community resources in Birmingham.
