East Sussex CAMHS - BAND 6 - Community Mental Health Practitioner



This is an exciting opportunity for a Mental Health Practitioner to join an enthusiastic CAMHS team in East Sussex.



The post is 37.5 hours per week.



This post offers the Trust's 2,000 Golden Hello or Relocation Incentive for Band 6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).



The successful applicant will be working with young people and their families who have been referred into CAMHS via SPOA (Single Point of Advice). The post will involve undertaking initial assessments and developing risk and care plans with young people.



You would also liaise with other professionals and be included in MDT meetings to discuss appropriate pathways for young people.



There will be some opportunity for brief solution focused interventions.



Contributing to the management of a team caseload of clients with complex needs, you will assess, plan, implement and evaluate interventions, which will be delivered mainly in the home or community setting. We follow a multi-disciplinary approach and as such you will provide specialist assessment and intervention with specific responsibilities regarding the acute care pathway.



We provide mental health and learning disability care for children, young people and adults across Sussex and for children and young people in Hampshire.

We manage some very complex cases but the team works well and provides lots of support and supervision and team members never feel they are working in isolation even during current remote working.

To provide specialist assessments and interventions for referred children and young people as part of a multi-agency approach to meeting the health and social care needs of children and young people who are referred to our services.



MAIN DUTIES



To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions. To support care co-ordination within the Tier 3 teams and attend relevant team meetings. To, provide specialist assessment and intervention as part of a multi-disciplinary approach. To support the Service Manger providing leadership and day-to-day management for junior staff, through supervision and appraisal. To support the Service Manager with, and to participate in, the planning, development and evaluation of local services within the Tier 3 CAMHS and to liaise with colleagues in Tier 4 services, as appropriate. To contribute to and facilitate evidence based assessments and individual and group interventions in a variety of situations. To contribute to the maintenance and development of the Service.

To regularly share and supervise students on practice placement, and junior staff as appropriate
