To actively track patients progress on a daily basis, throughout the patient pathway to ensure care is carried out in a timely fashion and follows escalation and PTL (Patient Tracking List) processes. Cancer Multi-disciplinary team (MDT) meetings: To ensure that all appropriate cancer patients are identified and discussed at MDT. Other: To facilitate all aspects of annual meetings, improvement meetings, internal review and National Peer Review as required. Please refer to the attached for further details.
