Modality LLP is a large-scale provider of NHS Outpatient Services, renowned for its innovative approach to delivering efficient pathways for patients requiring specialist care, leading to high-quality patient outcomes. We work in partnership with a number of hospitals across the country to collaboratively redesign outpatient services with more care being delivered in community settings.

We are looking for an experienced community Cardiographer who has the skills, knowledge, and experience to coordinate and provide holistic outpatient Cardiology investigation testing to patients. Both full-time and part-time opportunities are available.

Main duties of the job

Electrocardiograms (ECGs)

To lead the recording of routine electrocardiograms on adult and paediatric patients To clearly and concisely explain the procedure to the patient To perform the ECG accurately to the standard of the Society for Cardiological Science and Technology (SCST) To recognise normal and abnormal ECG records and report any abnormal findings to the appropriate clinical member of staff. To provide the correct documentation on the ECG within the patient's clinical record

Ambulatory ECG Monitors;

To prepare a range of monitors in preparation for the patient To provide clear and concise instruction to the patient on what the test entails, including advice on how to record information in the patient diary Apply the ambulatory monitor to the patient in accordance with the service protocol, ensuring all relevant documentation is completed To remove monitors from patients, ensuring that all symptom information has been noted in the patient diary To download and store holter monitors onto the appropriate computer programme, ensuring data is stored appropriately

Ambulatory BP monitors;

To issue and apply ambulatory blood pressure monitors to patients, ensuring clear and concise instruction is given to the patient relating to what the test entails and how to record any details in the patient diary To recognise the causes of erroneous recordings and to rectify where possible To download data into the appropriate database, ensuring accurate data management and to generate reports where appropriate To help maintain the high standards of the ultrasound service and to encourage working as a team with all members of staff including HCAs and reception staff

Over 15 years of experience of delivering community outpatient services Contracts with ICBs and hospital providers A diverse clinical workforce of consultants, GPs with extended roles, specialist nurses and allied health professionals Delivery of over 120,000 patient consultations per year Delivery of 1st consultations within a maximum of 4 weeks from referral from a choice of community locations, 7 days per week

Key Duties & Responsibilities

Clinical Responsibilities:

Other Duties:

Carry out cleaning and basis maintenance of equipment and clinical areas as required Ensure and maintain adequate stock levels. Deliver reports to clinicians in a timely fashion. To undertake regular audits for quality improvement of the service Carry out reception duties as directed. Deal with general enquires form staff, patients, visitors and member of the public in a helpful and courteous manner. Carry out responsibilities within one's own capabilities, qualifications and training Observe all security regulations currently in force when handling medical records and patient information. To undertake regular infection control of equipment in accordance with equipment manufacturer and service specifications To support the cardiac Physiologists working within the service in performing their role by assisting in various investigations/ procedures Maintain standards of conduct and dress in accordance to organisational policy Responsible for delivering clinics independently. Work as part of the multidisciplinary team fostering good interdepartmental relationships with a focus on continuous quality improvement. Document the details of all patient encounters/consultations within the clinical record and provide appropriate written information to support verbal advice. Communicate effectively with hospital medical staff, community staff, general practitioners and nursing staff giving specialist support and advice in the care and treatment of patients. Provide advice for other members of the healthcare team and patients as required. Actively participate in all aspects of the clinical governance process. Promote infection control, environmental safety and risk management in order to and minimise risk to patients, self and other staff at all times. Participate and support implementation of national guidance relevant to the service i.e. National Service Frameworks, NICE guidance etc Participate in team meetings.

Person Specification

Education/Qualifications

3 GCSE passes (grade C or above) or equivalent to include Maths, English Language and a Science

Level 2 Qualification in a Healthcare focused subject.

Knowledge

A minimum of 12 months' experience working with adult and paediatric patients

Experience of working in a patient centred role within a hospital/healthcare environment

A minimum of 6 months' experience of carrying out ECGs and holters and/or working as a HCA

Skills

Demonstrates clear commitment to the team approach, exchanging ideas and providing support to colleagues.

Able to work effectively in a team, supporting others and challenging colleague's views and attitudes when necessary.

Evidence of having worked with the public.

Able to convey instructions and ensure patient understanding clearly and appropriately.
