Job Description

We have an exciting opportunity within the Initial Response Team (IRT) Band 4 Care Act Assessor based in the Lichfield area.



This is an ideal opportunity for someone at the beginning of their Social Care career who wants to experience a wide range of opportunities in Adult Social Services. You will be fully supported to develop your skills and experience, and the successful applicant will be working alongside Social Workers, Social Care Assessors and Senior Care Act Assessors.



The role is vitally important in contributing towards the key performance indicators of the service. The role contributes towards ensuring safe, supportive and appropriate care which will support independence. The role is fast paced and involves working closely with service-users, carers, partner agencies and other professionals providing assessment of need, signposting, information, care planning and reviewing in line with The Care Act (2014) and Mental Capacity Act (2005).



Due to the nature of the role, you will be required to travel independently and efficiently across a large geographical area across Staffordshire and where appropriate, out of the County.



This is a full-time post, 9am-5pm, Monday to Friday. In the interest of a work/life balance, we may be able to offer some flexibility in hours, dependant on the service.



A good knowledge of the services available to support and promote independence in the community is a benefit but not essential as an induction and full training will be provided. The Trust is committed to professional development and will provide opportunity for training and routes for career progression.
