We are seeking a dedicated and compassionate Care Coordinator to support our children service (0-18)to join our team. The ideal candidate will play a crucial role in ensuring that our clients receive the highest quality of care and support. This position involves coordinating care services, liaising with healthcare professionals, and maintaining accurate records to facilitate effective communication among all parties involved in the care process. The Care Coordinator will work closely with clients, their families, and various healthcare providers to ensure that all needs are met efficiently and compassionately.
Responsibilities
Coordinate and manage care plans for clients, ensuring that all services are delivered in a timely manner.
Serve as the primary point of contact for clients and their families, providing support and guidance throughout the care process.
Liaise with healthcare professionals, including doctors, nurses, and social workers, to facilitate comprehensive care.
Maintain accurate and up-to-date records of client information, care plans, and service delivery.
Conduct regular follow-ups with clients to assess their needs and satisfaction with services provided.
Assist in the development of new programmes or services based on client feedback and emerging needs.
Ensure compliance with all relevant regulations and standards within the healthcare sector.
Participate in team meetings and contribute to the continuous improvement of care services.
Qualifications
Previous experience in a Child care service or social services setting is preferred.
Strong organisational skills with the ability to manage multiple tasks effectively.
Excellent communication skills, both verbal and written, to interact with clients, families, and professionals.
Proficiency in using office software for documentation and communication purposes.
A compassionate approach towards client care with a strong commitment to enhancing client well-being.
Ability to work independently as well as part of a collaborative team environment.
A relevant qualification in health or social care would be advantageous but is not essential.
