All Care Plus Ltd are a local, dynamic, friendly and well established domiciliary care company running for over 30 years.
We are seeking a dedicated and compassionate Care Co-ordinator to join our care team. The successful candidate will play a vital role in ensuring that our clients receive the highest quality of care and support. This position requires strong organisational skills and the ability to work effectively in an office environment. The Care Coordinator will be responsible for liaising between carers, clients, families, and healthcare providers to facilitate seamless care delivery.
Responsibilities
Ensuring the timely production, distribution and understanding of the rotas.
Fully manage and maintain the community carer workforce rota.
Ensure the rotas provide continuity for both carers and clients alike.
Ensure rotas are created in advance and calls fully covered maintaining 6 weeks in advance.
Work actively with the registered manager and senior care team to effectively allocate shifts and workload.
Produce weekly reports for the registered manager.
Liaise and build a positive relationships with carers and clients.
Work strategically to allocate carers to the correct geographical locations and clients to minimise any long and additional journeys.
Allocate calls allowing for the appropriate amount of travel time.
Work with the rest of the team to plan in advance for holidays and ensure contingencies are in place for sickness and unexpected absence.
Support the senior team to maintain up to date information on clients, updating tasks, care plans and risk assessments as necessary.
Conduct supervisions with the senior care team and provide support as required.
Ensure On Call cover is maintained in advance in line with the rota.
Work with the senior team to plan monitoring for the carers.
Support new carers with their induction, training and monitoring.
Actively undertake hands on care calls to cover unallocated calls as and when required and provide On Call cover as required.
Experience
Previous experience in a healthcare or social services setting is desirable.
Strong organisational skills with the ability to manage multiple tasks simultaneously.
Excellent communication skills, both verbal and written, to effectively interact with clients and professionals.
Proficient in using office software applications for documentation and scheduling.
A compassionate approach towards client care, demonstrating empathy and understanding.
