We are seeking a Care Co-ordinator to join our caring and dynamic team at Ladysmith Care Home, working 21 hours per week, working on a rota basis Monday-Sunday. You will report to the Care Home Manager / Management team. You must be committed and passionate in your care practises, you will also need to be knowledgeable in the standards expected within this framework.
To be fully aware of and adhere to all policies and procedures of the home.
To share information with other staff with regard to meeting the personal care needs of residents, in a way that respects the dignity of the individual and promotes independence.
Main Duties and Responsibilities:
To include residents and/or family members in the monthly review and up-date of their individual care plans, choices and preferences.
To take part in staff meetings and in training activities as required.
To report to the person in charge of any changes in a resident's condition.
To participate in the care planning and evaluation of residents and maintenance of relevant records.
To complete 'End of life' questionnaires with people at a time when the person feels happy to do this.
To contribute towards improving the standard of care of the residents.
To maintain confidentiality regarding all aspects of care and management of the home.
To perform any other such reasonable duties that may be required.

Licence/Certification:
Computer Skills essential
Experience in Electronic Care Plans preferably Person Centred Software (PCS) or equivalent.
NVQ Level 3 Health & Social Care (preferred)
