We are recruiting a full time Care Coordinator to work as part of our Multi-disciplinary Team!Working across Corsham and Box Community to coordinate patient care, the post holder is to provide time and support to patients, acting as a single point of contact to enable them to navigate the health care system. In this way, you will provide a more joined-up and co-ordinated care journey for the patient. You will be expected to work closely with the GPs and other primary care professionals within the PCN, to identify and manage a caseload of patients, making sure that appropriate support is made available to them and their carer and ensuring that their changing needs are addressed.Based at The Porch Surgery Corsham, youll join a supportive team, working together to provide outstanding service to our patients and clinical teams. You will have the flexibility to work within a model that supports both on-site and hybrid working arrangements, depending on the nature of the role.
