Job Description

The post holder will work with the Management team and relevant teams and practices to support coordination of services within the network.

The post holder will work closely with staff working at practice and PCN level to understand current service models, identify challenges, agree priorities for improvement and then support these to be delivered. The post holder will develop strong relationships with PCN member practices and will support the planning and delivery of improvements that will provide benefits to patients and staff.

The post holder will work with clinical and operational teams within a Primary Care Network (PCN) to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the inspiration, resource and tools to deliver changes in a collaborative way. The post holder will support this vision to be developed in alignment with ICS strategy.

The post holder will require knowledge and skills in Data Management and will support the PCN leadership team in monitoring performance and standards across the network.

The post holder will be an integral part of the general practice team, working as and when required in each organisation and across the PCN.

The role will be employed under the Additional roles Reimbursement Scheme (ARRS) meeting the requirements outlines in the Network Contract Agreement DES.

The following are the core responsibilities of the PCN Care Co-ordinator:

There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Provide hands-on capacity and support to practices in implementing and sustaining agreed changes.

b. Support practices and staff to effectively use existing and new technology systems and products to enable improvement.

c. Advise on innovative opportunities and support practices to maximise benefits from innovation, through the sharing of learning and best practice.

d. Provide feedback to PCN leadership and to member practices on progress of current initiatives, capacity for change, progress and issues so that plans can be adapted in response.

e. Take a data driven approach, looking at evidence and information available to support understanding of challenges, identify possible solutions and track the impacts of changes that have been made.

f. Bring together data from a variety of sources and use information to develop insights into working practices, effectively tailoring content to meet the needs of the audience.

g. Effectively present and communicate data to practices and PCN staff at all levels to increase understanding of operational challenges

h. Interpret data and highlight variation to support open discussion about where variation is and is not warranted

i. Help PCN and practice staff understand the limitations of particular data sets and where other information may be available to supplement understanding of issues

j. Use data to track and assess impact of changes made to operational processes, and to track trends and developments over time.

k. Ensure that changes are aligned with agreed clinical priorities and have ownership and engagement from clinical leaders and staff.

l. Identify best practice and learning from other areas that could be applicable to the local PCN and communicate this to staff.

m. Make connections across the PCN to support learning between practices and sharing of best practice.

n. Lead or participate in relevant working groups, project groups, service meetings and other forums across the PCN (and ICS) to agree changes, share learning, discuss challenges, and resolves issues.

o. Support practices with NHS Contract work, Including the LES and the DES.

p. Monitor and provide regular updates to practices about their progress in project work and national contracts

q. Create searches in the clinical system to provide support and monitoring of practices achievement.

r. Communicate at all levels across organisations ensuring that an effective, person-centered service is delivered.

s. Attend meetings with the PCN when required, to provide updates and discuss the service.

t. Undertake all mandatory training and induction programmes

u. Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.

v. Maintain a clean, tidy, effective working area at all times.

In addition to the primary responsibilities, the PCN Care Co-ordinator may be required to:

a. Support the delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives across the network

b. Support PCN staff, providing cover during staff absences

c. Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.

d. Duties may vary from time to time without changing the general character of the post or the level of responsibility.
