Job Description

To work closely with teams to help and support the multi-disciplinary team (MDT) meetings and the ongoing patient case management. This will involve linking with GP surgeries and a range of Community Health Services
Main duties of the job
Support the MDTs within the Primary Care Network (PCN) to deliver effective, co-ordinated care for vulnerable and frail adults, particularly those at high risk of a hospital emergency admission, emergency department (ED) attendances or out of hours care

The employer for this vacancy will be Connect Healthcare Rotherham Community Interest Company (CIC) is a federation of 28 GP practices in Rotherham. The organisation is led by a Board of representatives from general practice in the Borough, with executive leadership from a Medical Director on a part-time basis. However this post will be on behalf of and working with Doncaster East PCN
