Job Summary

The post holder will work within the PCN Team, providing coordination, administrative and support to the Clinical teams and other members of the PCN.

Ensure an effective and efficient performance overall for the Practice. Assistant the Business Manager on Clinical, Practice Claims, Research, and Practice Systems. Using your own judgment and communication skills ensure that performance within the Practice are a high priority. Optimising overall performance and ensuring the practice achieves its long-term strategic objectives in a safe and effective working environment.

Duties:

v Work on own initiative with minimal supervision daily and oversee the day to day

management of the team.

v Ensure that resources and processes are coordinated to deliver successful operational

v Pro-active management of changing priorities to include the co-ordination of daily cover

across the team/s always maintaining service levels.

v To be first point of contact when dealing with

v Ensuring clinical session structure is effects, meets the need of the patient and clinicians have access to suitable rooms.

v To have a comprehensive knowledge of all Practice agreed SOP and delivery of the key performance indicators (KPIs) which have.

v Chaperoning duties

v To work alongside the Leadership Team and Business Manager validating patient pathways on

regular basis through use of information dashboards, maintaining a high degree of accuracy

for prolonged periods of time

v Involved in the planning and implementation of new business process and contribute towards on-going service improvement.

v Undertake an active role in research projects relating to the development and evolution of the Practice Research.

v Reviewing and updating clinical templates ensuring they relate to current practice.

v Implementation and update care navigation system on clinical system.

v Assist Business Manager with Practice Claims from external stalk holders such as CCG & PCN.

v Provide Extend Access administration and co-ordination to ensure Practice is optimising its appointment systems.

v Support Digital & Transformation Manager with QOF, IIF and enhanced services ensuring rota are fully booked to ensure high achievement

v To lead on Practice's Des, Les and QOF Performance, providing updates and action plans to ensure the Practice fulfils its targets and KPIs.

v Supporting the overall practice clinical framework, submitting reports for QOF, enhanced services and other reporting requirements using CQRS/ Open Exeter and other external software.

v Monitoring and disseminating information on safety alerts and other pertinent information.

v Co-Ordinator and action Practice audits to continue to improve Practice Performance and patient care.

v Co-ordinator and minute Practice Meetings structure, local MDT and Safeguarding to ensure the Practice information is shared through communication with staff and local stalk holders.

v Be the point of call for staff to raise any System Online patient accounts.

v Report any issues or complaints as necessary to the Business Manager.

v Assistant Practice Manager on Health & Safety Regulations and to be a point of call to report any concerns to NHS Properties.

v To work as Team with the Digital & Transformation Manager, Reception Team Leader and Business Manager to ensure the Practice is running most effectively.

v Work flexibly and effectively within the Practice teams so that essential priority tasks are

covered and dealt with.

v To promote good team working conditions and understand the needs of the other team

members.

v To follow relevant procedure in calling for the appropriate level of assistance in an emergency.

v To work in accordance with Practice policies and guidelines at all times.

v To always adhere to Practice Information Security Policy, in particular ensuring there are no breaches of confidentiality.

v Other duties as may be required in accordance with this grade of post.

v The post holder will work closely with Care Homes, General Practices and existing services to support the coordination and delivery of multidisciplinary team meetings for care home residents.

Personal Specification

Experience

Essential

Experience of dealing with General Practice, Performance, QOF, Systems and Process.

Desirable

Health Care Management Qualification.

Qualifications

Qualifications A Levels grades in English & Maths Qualified to NVQ level 3.

Pay: 13,520.00 per year
