The Role
As a
Care Coordinator
, you will work as a strong team player within a team of Care Coordinators who collectively play a key role in both
care delivery and people coordination
. Alongside managing care schedules and supporting clients, you will be actively involved in
recruitment, onboarding, and day-to-day HR support
for our care team.
This is a varied and rewarding role, ideal for someone who enjoys working with people, managing processes, and contributing to an OUTSTANDING service.
Key Responsibilities
Care Coordination
Coordinate and manage care rotas to ensure continuity and quality of care
Match carers to clients based on skills, experience, and preferences
Act as a main point of contact for clients, families, and care staff
Respond proactively to changes in care needs, staff availability, and emergencies
Maintain accurate care records and support regular care plan reviews
Liaise with healthcare professionals and external stakeholders
Recruitment & HR Support
Support the recruitment of care staff, including advertising roles, screening applications, and arranging interviews
Coordinate onboarding, inductions, and compliance checks (DBS, right to work, training records)
Maintain accurate staff records in line with GDPR and CQC requirements
Provide day-to-day support to care staff, including absence reporting and basic HR queries
Support performance management processes, including supervision scheduling and probation reviews
Assist with staff retention initiatives and promote a positive, supportive workplace culture
About You
Experience in
home care, care coordination, recruitment, or HR support
(preferred)
Highly organised with strong attention to detail
Confident communicator with excellent people skills
Able to manage competing priorities in a fast-paced environment
Comfortable using care management systems and HR/admin software
Passionate about delivering OUTSTANDINGcare and supporting care teams
Essential Requirements
Right to work in the UK
Enhanced DBS
Excellent team worker
Excellent understanding of person-centred care
Caring and compassionate, with a positive can-do attitude
Experience, and a genuine desire, in providing home care first-hand
Flexibility to support on-call duties
Desirable (but not essential)
NVQ Level 3 in Health & Social Care (or working towards)
Experience within a CQC-regulated service
Recruitment, onboarding, or HR administration experience
