Your Role as a Care Coordinator:

As a

Care Coordinator

, you'll play a key role in delivering and managing exceptional home care services. You'll split your time between our Cardiff office (3 days per week) and the community (2 days per week), providing direct care, conducting staff assessments, and updating care plans.

You'll also share responsibility for our

on-call rota

, ensuring consistent support for clients and carers.

What we are looking for:

Previous experience as a

Care Coordinator

,

Field Care Supervisor

, or

Senior Carer

Strong organisational and communication skills Confident in using

care planning software

and

Microsoft Office

Understanding of

health and social care standards

and

CIW regulations

Ability to work under pressure and prioritise effectively A genuine passion for providing outstanding care in the community

Full UK driving licence

and access to your own vehicle

Key Responsibilities:

Coordinate and manage

high-quality domiciliary care services

Liaise with carers, clients, and families to ensure

individualised care delivery

Conduct

staff supervision

,

competency checks

, and

client care reviews

Create and update

person-centred care plans

using our care management system Ensure compliance with

CIW regulations

and

company policies

Manage day-to-day care queries and handle emergency situations effectively Maintain detailed, confidential care records and documentation Participate in the

on-call phone rota

as part of your full-time duties
