Duties
Coordinate care plans by collaborating with healthcare providers, clients, and families to ensure comprehensive support.
Maintain accurate records of client interactions, care plans, and progress notes.
Schedule appointments and follow-up visits for clients, ensuring timely access to necessary services.
Act as a point of contact for clients and their families, addressing any questions or concerns regarding care.
Monitor client progress and report any changes in condition to appropriate healthcare professionals.
Assist in the development and implementation of care strategies tailored to individual client needs.
Provide administrative support within the office environment, including managing correspondence and maintaining files.
Skills
Proven office experience is essential, demonstrating strong administrative capabilities.
Excellent communication skills, both verbal and written, with the ability to convey information clearly and compassionately.
Strong organisational skills with attention to detail, enabling effective management of multiple tasks simultaneously.
Ability to work collaboratively within a team while also being self-motivated to take initiative when required.
Proficiency in using office software applications for documentation and communication purposes.
