The Care Coordinator will work as part of the pharmacy team to support the delivery of safe, effective, and coordinated medicines-related care for patients across the network. The postholder will play a key role in improving patient experience, ensuring smooth communication between patients, the pharmacy team, general practice clinicians, and community or hospital pharmacy services.
This role is ideal for someone who enjoys patient contact, excellent organisation, and collaborative working within a multidisciplinary team.
Key Responsibilities
1. Patient Coordination and Support
Coordinate medication reviews, structured medication consultations, and follow-up appointments.
Support patients with medication queries, repeat prescription requests, and medication adherence plans.
Help identify patients who would benefit from pharmacy team input (e.g., polypharmacy, high-risk medicines, recent discharge).
Assist with communications following hospital discharges to ensure timely medicines reconciliation.
2. Communication and Liaison
Liaise between GP practices, community pharmacies, care homes, hospitals, and other healthcare professionals to ensure continuity of care.
Maintain accurate patient records and ensure documentation is completed promptly in line with practice protocols.
3. Administrative and Project Support
Support the pharmacy team with administrative tasks such as scheduling reviews, managing shared inboxes, and maintaining spreadsheets or EMIS templates.
Assist in data collection and reporting for pharmacy services and IIF/QOF indicators.
Support audits, medicines optimisation initiatives, and patient education projects.
Skills and Experience Required
Essential
Excellent organisational and administrative skills.
Strong communication and interpersonal skills.
IT literacy, including experience with clinical systems (EMIS) and Microsoft Office.
Understanding of confidentiality, data protection, and safeguarding principles.
Desirable
Experience working in a GP practice, PCN, or community pharmacy setting.
Knowledge of medicines management and the role of pharmacy teams in primary care.
Experience supporting patients with long-term conditions.
Familiarity with NHS structures, referral processes, and care coordination.
