Job Overview


We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will play a vital role in ensuring that patients receive the appropriate care and support they need throughout their healthcare journey. This position involves liaising with patients, families, and healthcare professionals to facilitate effective communication and coordination of services.

Duties



Serve as the primary point of contact for patients and their families, addressing inquiries and concerns regarding care plans. Coordinate appointments, referrals, and follow-up services to ensure seamless patient care. Maintain accurate and up-to-date patient records, ensuring compliance with confidentiality regulations. Collaborate with healthcare providers to develop and implement individualised care plans tailored to each patient's needs. Monitor patient progress and communicate any changes or concerns to the relevant healthcare team members. Assist in the development of educational materials for patients and families regarding available services and resources. Participate in team meetings to discuss patient cases and improve care coordination processes.

Skills



Strong organisational skills with the ability to manage multiple tasks efficiently. Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare professionals. Proficiency in office software applications for documentation and communication purposes. Empathy and compassion towards patients, demonstrating a commitment to providing high-quality care. Ability to work collaboratively within a multidisciplinary team environment. Attention to detail in maintaining accurate records and documentation.

Job Type: Full-time

Pay: 24,820.00-25,652.00 per year
